Business Honor Roll

The Business Honor Roll of Commonwealth Health Foundation was established to recognize businesses that support the advancement of healthcare in Southcentral Kentucky. Funds raised through the Business Honor Roll will provide financial assistance to support the mission of the Foundation.

Lifetime membership in the Business Honor Roll is awarded to individuals who commit $10,000 or more to Commonwealth Health Foundation. This may be fulfilled by one single payment, in annual payments over a period of five or less years, or a payment schedule that meets your needs.

Eligibility

This select group of businesses and industry leaders provide funds not otherwise available for making significant improvements in the quality of healthcare in our communities. Your partnership is advantageous to both your company and the Foundation. When you support the Foundation, you are not only assisting the healthcare community – you are helping yourself, your employees and your fellow neighbors.

Your donation will be calculated cumulatively and will include all gifts (excluding ticket purchases) made to the Foundation: pledge payments, unsolicited gifts, tribute gifts, event sponsorships, fair market value of auction items donated, amount paid for auction items, and in-kind donations.

Recognition

Commonwealth Health Foundation extends special recognition to this select group of businesses and industry leaders.
Recognition in various health system publications.
Recognition at an annual social event where new members will be honored and presented with a personal gift of appreciation.
Special invitations to CHC and Foundation functions, such as dedications, open houses and other special events.

All gifts are tax deductible as allowed by current law. The Foundation recommends that you check with your personal tax advisor as to the deductibility of your gift.
